Learning from Case Reviews

Learning from Child Safeguarding Practice Reviews (CSPR)

TSCP is committed to improving outcomes for children and young people through learning and improvement activity.

TSCP adopts and promotes a “learning culture” whereby we seek to learn from every opportunity. The case review process does not seek to apportion blame to individuals or agencies, rather the focus is on identifying learning that will change our system responses, and in turn lead to improved outcomes for children and young people.

When a child dies or suffers serious harm, and abuse or neglect is suspected, TSCP will undertake a review of multi-agency safeguarding practice (Working Together to Safeguard Children, 2023). Our local model also allows for reviews of cases that may not meet the statutory criteria.

The purpose of any review is to elicit new learning that will improve and strengthen our local safeguarding system. All case reviews undertaken by TSCP will seek to:

  • Establish whether there are any lessons to be learnt from the circumstances of the case, about the way in which professionals and agencies work together to safeguard vulnerable children, young people and their families
  • Review the effectiveness of procedures or the commissioning of services
  • Inform and improve local multi-agency safeguarding practice approaches
  • Ensure that the workforce development offer is robust and effective
  • Promote and highlight good practice

Published reviews are available to download on TSCP website for 12 months. 7-minute briefings and bitesize briefing sessions are accessible for all partners to support learning and improvement.
